Erythrodermic psoriasis appears with a generalized reddening of the skin. It is a very serious skin condition that can be fatal!

It can be the result of many inflammatory conditions of the skin, drugs and malignant tumors, but in one third of cases it is due to psoriasis.

Erythrodermic psoriasis usually occurs with worsening or instability of psoriasis, but can be extraordinarily the first presentation of psoriasis.

Erythrodermic psoriasis can be precipitated by:
  • Infections.
  • Low calcium.
  • Withdrawal of oral corticosteroids (prednisone).
  • The withdrawal of excessive use of strong topical corticosteroids.
  • Strong preparations of coal tar.
  • Certain medications such as lithium, antimalarials and interleukin.
  • Excess consumption of alcohol.


Complications include:


  • Dehydration.
  • Heart Failure.
  • Infection.
  • Hypothermia.
  • Loss of protein and malnutrition.
  • Edema (swelling), particularly of the lower legs.
  • Death.


Treatment - Erythrodermic Psoriasis Treatment!


Treatment of erythrodermic psoriasis can be difficult. Management includes:


  • Hospitalization for supportive care including intravenous fluids and temperature regulation.
  • Applying emollients and wet dressings.
  • Bed rest.
  • Treatment of complications (eg, antibiotics, diuretics (urine tablets), nutritional support).
  • Low doses of methotrexate, cyclosporine or acitretin.
  • Individual cases have been reported to have successful results with biological agents, including TNF-alpha, which are the inhibitors adalimumab, etanercept and infliximab, and ustekinumab.


Oral corticosteroids should be avoided, if possible, because the risks of withdrawal worsen the condition of erythrodermic psoriasis or generalized pustular psoriasis. However, sometimes it is the only treatment that helps.

Topical tar preparations and phototherapy should also be avoided in the early treatment of erythrodermic psoriasis, as they may worsen the condition.
