It seems strange that psoriasis, a disease caused by an hyperactive immune system, can occur in people with the human immunodeficiency virus (HIV), a disease that destroys the immune system.

Psoriasis occurs in about 3% of people with HIV, the same rate as in the general population. It is a chronic disease that results from an excessive growth of skin cells that form red and scaly lesions on the skin. Psoriasis is characterized by periodic outbreaks caused by stress, illness, injury or allergies.

Outbreaks tend to be more severe in people with HIV, and increase in severity when HIV infection progresses.

Standard treatment.

Do not rely on treatments for standard psoriasis. They can be ineffective for people with HIV.



  • Uses creams with topical corticosteroids and ointments for mild outbreaks.
  • Cleans skin lesions with phototherapy, treatment with ultraviolet (UV) B rays.
  • Treat more severe outbreaks with anti-TNF (tumor necrosis factor) inhibitors, such as etanercept (Enbrel) and infliximab (Remicade). Anti-TNF inhibitors require regular injections and may be associated with side effects that include tuberculosis and other infections.


Avoid methotrexate and cyclosporin. Although these are used to treat psoriasis, they can not be used for people with HIV infection because they suppress the immune system.

It slows cell growth with oral retinoids (acitretin) plus topical retinoids (calcipotriol or tazarotene). It controls the production of lymphocytes with UVB light plus topical retinoid (calcipotriol or tazarotene).

Another treatment to control lymphocyte production is psoralen and UVA light (PUVA). It is used with topical or oral retinoids.

Make sure that the patient with HIV and psoriasis follows highly active antiretroviral therapy for HIV infection. Studies have shown that antiretroviral therapy effectively controls outbreaks of psoriasis, making it doubly important for patients with HIV and psoriasis to continue treatment without disruption.

Non-medical therapies.

Keep the skin moisturized to control the itching that accompanies psoriasis outbreaks.
Relieves discomforts with cold water packs, cold showers, antihistamines, topical corticosteroids and antihistamines.

Explore alternative therapies. The Astragalus, is a root used in traditional Chinese medicine, contains the chemical TAT2. Research has shown that TAT2 improves the ability of cells to fight HIV infection.

Indigo naturalis, a herbal powder, is another ingredient used in Chinese medicine to reduce skin flaking and redness.
